Co-payments are due at time of service. If you are a self-pay patient, you will be charged a flat fee which is approximately 50-70% of what is customary. Payment is expected in full at the time of service unless other arrangements are made.
In-house lab service is provided by Labcorp. Lab work is done by appointment only. Lab work will be billed separately by Labcorp. You may also have your labs done by a lab of your choice or by your own physician.
Patients are seen by appointment only. Please arrive at least fifteen(15) minutes prior to your appointment. Please have your co-pay and your insurance card(s) ready to give to the receptionist. You will be asked to confirm your address, email, and phone number.
You will receive an automated appointment reminder by text, email or phone one week and three days prior to your appointment. Please confirm or call to reschedule as soon as possible should you need to change your appointment.
A referral is required for new patient appointments. Please try to arrive thirty(30) minutes prior to your appointment. Your physician can call or fax a referral to our clinic. The referral should include your demographic and insurance information, recent lab work, your last office visit note and any other relevant tests.
